Is there any co-insurance for hearing aids, if yes, what is the split?

Is there a maximum allowed cost associated with each hearing aid that the provider can bill them?

Are you required to find an in-network provider? Can you go also with an out-of-network provider? They probably have different co-insurance ratio for in-network vs out-of-network.

Is there a particular managed care provider (like TruHearing or Amplifon for example) who has a contract with your insurance company whom you are required to purchase the hearing aids through them only? Or can you go to any in-network provider even if they don’t sub-contract through that managed care provider?
