I have Blue Shield Promise health plan.
Has anyone purchase from Costco Hearing center and got reimbursement for their Hearing Aids?
Did you get reimburse fully?
If so, what is documentation is required from Costco?
I assume it would be the itemized purchase, test results and form from your insurance provider that you need to fill out.

I am in the process of getting reimbursed through Aetna. All they asked me for was a copy of the receipt.

PPO…Costco is out of network
I think your best bet is to call your insurer and read your policy. My insurance, administered by BCBS of Illinois, paid me what I expected.
